But before you book that European river cruise or fly to visit family overseas, there's one critical thing most travelers on Medicare don't discover until it's too late: in almost every situation, Medicare does not cover you outside the United States.
That's not a footnote. That's a financial exposure that can turn a dream trip into a six-figure medical bill. A single hospitalization abroad — a heart attack, a fall, an emergency surgery — can cost tens of thousands of dollars that Medicare will not pay for. Understanding exactly what is and isn't covered before you travel could be the most important financial decision you make this year.
Original Medicare — Parts A and B — covers hospital and medical services inside the United States and its territories (Puerto Rico, Guam, U.S. Virgin Islands, American Samoa, and the Northern Mariana Islands). The moment you cross into Canada, Mexico, Europe, Asia, or anywhere else, your coverage does not travel with you.
This surprises a staggering number of retirees. Many assume that because Medicare is a federal program, it follows them wherever they go. It doesn't. The limitation is written directly into Medicare law, and there are only three narrow exceptions where Medicare may pick up the bill for care received outside U.S. borders.
If you receive medical care abroad and expect Medicare to cover it, you will almost certainly receive a denial — and be responsible for the full cost out of pocket. Do not travel internationally without understanding your coverage gap and having a plan to fill it.
Medicare does recognize three specific circumstances where it will cover care received outside the United States. These exceptions are narrow and come with conditions — but they're worth understanding, particularly if you live near a U.S. border or cruise frequently.
Outside of these three scenarios, Original Medicare coverage simply does not apply. No reimbursement. No partial coverage. No safety net. Which is why understanding your supplement and advantage options is so critical before international travel.

Here's a straightforward breakdown of what Original Medicare covers and doesn't cover when you're traveling internationally — so there's no ambiguity when you're planning your trip.
This is where the picture gets more nuanced — and more hopeful. While Original Medicare won't cover you internationally, some Medicare Advantage plans include emergency coverage outside the United States as an added benefit. Coverage varies significantly by plan and carrier, so this requires careful review before you travel.
Most Medicare Advantage plans that include international emergency coverage will cover urgent and emergency care abroad — typically with a copay or coinsurance — but generally do not cover routine or elective care outside the country. Some plans cap international emergency benefits at a set dollar amount per year (often $50,000–$100,000), so knowing your plan's limits is critical.
For retirees who travel internationally with any regularity, a Medicare Supplement plan — also called Medigap — is widely considered the gold standard for filling the foreign travel gap. Certain Medigap plan letters specifically include a foreign travel emergency benefit as a standard feature.
Medigap Plans C, D, F, G, M, and N all include a foreign travel emergency benefit. These plans cover 80% of medically necessary emergency care received outside the U.S., after a $250 annual deductible, up to a lifetime maximum of $50,000. It's not unlimited coverage, but for most medical emergencies abroad, it provides meaningful and reliable protection.
Medigap's foreign travel emergency benefit has a $50,000 lifetime maximum. Medical evacuations alone can cost $50,000–$200,000. If you're a frequent international traveler, consider pairing your Medicare Supplement plan with a separate travel insurance or medical evacuation policy for full peace of mind.
Even if you have some form of emergency medical coverage when traveling internationally, your Medicare Part D drug coverage almost certainly doesn't travel with you. Medicare Part D covers prescription drugs dispensed at U.S. pharmacies. Prescriptions filled at a foreign pharmacy are not covered under any standard Part D plan.
This matters more than most people realize. If you take daily medications and run out while abroad, or need a new prescription from a foreign doctor, you will pay out of pocket. Some Medicare Advantage plans include limited prescription benefits for travel emergencies, but these are the exception rather than the rule.
The coverage gap for international travel is real, but it's entirely fillable. The right combination of plan type and supplemental coverage can give you genuine peace of mind on any trip — whether you're crossing into Canada for a weekend or spending three months in Southeast Asia.
Retirement travel is one of life's great rewards. But walking onto an international flight assuming your Medicare coverage is with you is a mistake that can cost more than the trip itself. Original Medicare is clear: it doesn't follow you abroad. And the financial exposure — tens or hundreds of thousands of dollars for emergency care and evacuation — is not something most retirees can absorb.

A Medicare Supplement plan with foreign travel benefits, a Medicare Advantage plan that includes international emergency coverage, or a dedicated travel insurance policy can each provide meaningful protection.
